Waiver /Disclaimer: As a participant
I am at least 18 years of age and understand that I am solely responsible for
any illness or injuries that I may incur during the workshop and that I am
responsible for all resulting medical costs or other expenses. Ancient Arts®
has permission to use any photos taken of me during the workshop for educational
or promotional purposes.

REGISTRATION
IS INCOMPLETE WITHOUT SIGNATURE
*Your
tuition is refundable less a $50.00 administration fee if written
cancellation notice is emailed or postmarked 45 days prior to the
scheduled workshop. After that time it is non-refundable unless the workshop is
canceled or your space can be resold.
Any non-refundable unused tuition may be applied toward a future workshop.
** If using PayPal Payment Option your
workshop space will be held for business 10 days, however, your registration is
not complete until your signed hard copy registration form is received
through U.S. mail. If it has not been received within 10 business days you may
forfeit your space.
Lodging and meals are not provided. Limit one
non-participant companion (observer) per paid registrant. No children or pets
please.
Upon receipt of the registration form and
payment, you will receive an email confirmation.
Approx. 30 days before event, a course schedule and other related workshop
information will be emailed to you as it becomes available.
